[Centres] Centre Support Self Assessment link

Hi all, 

As we start to get Centres back up and running please review the Centre
Support Scheme requirements CEN-09B and complete the Self-Assessment at the
following link https://www.surveymonkey.com/r/6G8PR67. 

This should be completed and submitted within 6 weeks of commencing programs
in 2021. 

If you have any questions please don't hesitate to ask.
