Attached please find:

 

1.	Cover letter to Centres for Director Nomination and Candidate
Profile Form;
2.	Director Nomination and Candidate Profile Form - pdf version - for
completion via hand writing; and
3.	Director Nomination and Candidate Profile Form - Word version - for
completion via computer (has expandable fields where necessary).

 

Please ensue that your Centre's Members are aware of these documents and
that they are encouraged to consider being nominated to the Board.

 

Also, please ensure that the Centre and any nominee carefully read and
comply with the requirements for nominations.

 

Nominations are to be received at State Office no later than the date that
is stated on the Forms.  This date is important in order for the timetable
provisions of the Constitution to be complied with.

 

To ensure that nominations are not subjected to possible postal delays and
that they are received at State Office by the due date, it is suggested that
consideration be given to, wherever possible, the applications being emailed
to State Office at the email address on the CC line to this email.  However
postal submissions are permitted and are to be sent to:
