[Centres] Coaching and CD Personnel Registration for 2022

Hi all, 

Another COVID affected year has made it difficult for Centres and Coaches to
meet the standard requirements for CD and Coaching Qualification Maintenance
therefore this year we have again adopted a group registration form for
Centres to complete. 

Please complete and return the attached ASAP to register and Coaches and CD
Personnel for 2022. 

It is important to also advise us if there are trainees that have left or
have decided not to continue training. 

If you have any questions please don't hesitate to ask.
