[Centres] 2024 ESR-01 Forms

Hi all, 

Please find attached the 2024 ESR-01 Forms for all Participants. 

There has been significant changes to Section 1: Participant Consent form
which will need to be completed prior to programs commencing in 2024. 

If the Participant has a Section 2: Medical consent section completed within
3 years it is the Coaches discretion to have this part updated however it
must be completed by the 3 year anniversary regardless of the date of the
Section 1: Participant Consent section. 

If you have any questions please don't hesitate to ask. 

It has been updated in the Members area also.
