[Centres] Information requested by insurer

Centre Secretaries.

 

I understand that some Centres have not been able to open up the document
that I sent out asking for information that has been requested by the
insurer.  The insurer has requested the information in order to determine
risk and issue the insurance policies for 2022.  Attached are Word versions
of the covering letter and the questions that need to be answered.

 

It is not possible to fill out the information on the attached document.
Therefore could you please reply to this email (reply to both State Office
and to me) giving answers, in the body of the email, to the questions that
apply to your Centre.

 

When replying, please made sure that you keep the same subject heading so
that I can readily locate your response.
